Float Case Manager

Humana seeks a Float Case Manager to support members across multiple programs, stepping in where needs are greatest. In this dynamic role, you will manage a rotating caseload, perform telephonic and virtual assessments, and create individualized care plans that integrate medical, behavioral, and social supports. You will coordinate benefits, providers, and community resources, monitor outcomes, and document all activities in care management tools.

Responsibilities

• Manage a rotating caseload of members across lines of business as a float resource
• Conduct telephonic and virtual assessments to identify medical, behavioral, and social needs
• Develop and update individualized care plans in collaboration with members and providers
• Coordinate services, benefits, and community resources to address gaps in care
• Monitor outcomes, track progress, and adjust interventions as needed
• Document all activities accurately in care management systems and electronic records
• Collaborate with interdisciplinary teams, including nurses, social workers, and physicians
• Educate members and caregivers on benefits, care plans, and condition self-management
• Escalate high-risk cases and support transitions of care to reduce avoidable admissions
• Maintain compliance with regulatory, accreditation, and Humana policies and quality standards
