Case Manager

Humana is seeking a Case Manager to support members in the La Crosse/Sparta area through comprehensive care coordination. In this role, you will assess medical, behavioral, and social needs; develop person-centered care plans; and coordinate services across providers, community resources, and benefits programs. You will monitor progress, close care gaps, and advocate for members to improve health outcomes. Working in a collaborative, member-focused culture, you will use digital tools, data, and clinical or social work expertise to deliver accessible, personalized support in a hybrid field/remote model.
