Center Well Home Health is seeking a dedicated LPN/LVN to provide one-on-one nursing care in patients' homes. Reporting to the Clinical Manager, you will implement care plans, educate families, and document visits to support recovery and independence.
You will work closely with a team of physicians and clinicians, coordinate with the care team, and help patients regain their life and confidence s role requires strong bedside skills and compassion.
