Salary Range: $19.00 - $25.50
- Provide resident-centered nursing care and daily living assistance to assigned residents under the supervision of a registered nurse (RN).
- Deliver age-appropriate care related to the physical and psychological needs of the resident as per the care plan.
- Follow infection prevention and control policies and use of Personal Protective Equipment; adhere to facility standards.
- Assist with basic health care needs including bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support.
- Assist residents in transferring, repositioning, and walking using correct techniques and equipment; provide range of motion and passive exercises.
- Assist with food preparation and dining to create a positive dining experience; help with meals and snacks and provide water and supplements as appropriate.
- Assist providers and nursing staff with procedures as needed; document resident interactions and outcomes related to care, behavior, activity, and dietary needs.
- Communicate changes in the resident's condition and care-related concerns to the charge nurse.
- Collect non-invasive body fluid specimens when required and take vital signs.
- Provide housekeeping and laundry services to maintain clean and operational living areas.
- Support wellness and recreational programs and encourage resident participation.
- Participate in departmental or organizational committees and task forces as needed.
- High school graduate or GED preferred, but not required.
- Minimum age 16; prior clinical or nursing assistant experience in long-term care preferred.
- Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state-certified CNA within four months of hire (enrolled in a training program or in the process of certification through Endorsement). CNA training and certification provided by the facility.
- Completion of yearly department-specific competency testing and maintenance of continuing education credits (minimum 12 hours annually) and department-specific certifications.
- In applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry when required.
- May classify as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver’s license; must be medically qualified to perform essential driving functions per Sanford Driving Policy upon leadership request.
