Licensed Practical Nurse (LPN) – Multiple Locations (Aurora, Chicago (Oakbrook Terrace), IL, USA's IL‑Joliet)
Position SummaryProvides direct and individual nursing care to assigned residents based on nursing standards and under the supervision of the Director of Resident Care (DRC) or the Health Care Coordinator (HCC). Maintains compliance with all state and federal regulations and the standards of HRA. Eligible for full-time employment with a regular start time for shifts (11pm‑7am, 8:30am‑6pm, 7:30am‑5pm, 11pm‑7am, 5am‑9pm weekdays, 6am‑5pm weekends).
Transportation, shift flexibility, and parital shift options are available for some locations.
- Administer medications and treatments; document accurately in EHR or MAR.
- Take vital signs, monitor resident condition, and report incidents or changes promptly.
- Assist with resident assessments, nursing calls, and incident reporting.
- Provide personal care: bathing, dressing, feeding, walking, transferring.
- Collect lab samples and follow up on results with residents, families, and physicians.
- Maintain cleanliness and safety in resident rooms; respond to resident calls.
- Support and train staff as directed by the HCC or DRC.
- Comply with HIPAA, company policies, and federal/state regulations.
- Work collaboratively with multidisciplinary teams and household staff.
- Assist in planning and implementing care plans, daily medication administration, and documentation.
- Support residents in self-care activities and monitor progress.
- Ensure confidentiality and resident rights regarding personal information.
- Perform any other duties requested by supervisor.
- Graduated from an accredited LPN/LVN program and currently registered in the state of practice.
- Valid continuing education credits as required by the state.
- Experience in Alzheimer and Dementia, long‑term care, or geriatric nursing facilities preferred.
- Minimum of one (1) year nursing experience in a health care facility.
- Experience in emergency care, first‑aid, and with Microsoft Office/Outlook.
- Ability to communicate effectively with all levels of management, residents, family, and outside contacts.
- Certified in CPR (AHA) or willingness to obtain certification within 2 weeks of employment.
- Physical ability to lift up to 50 lbs, stand, bend, walk, and assist residents.
- Passion for working with senior citizens and providing compassionate care.
- Background screening:
Level 2 in Florida or comparable state clearance for all US positions.
