Travel Physical Therapist (PT) – Skilled Nursing Facility (SNF)

We are seeking a qualified Travel Physical Therapist (PT) for an assignment in a Skilled Nursing Facility (SNF). The Physical Therapist will evaluate and treat patients to improve mobility, reduce pain, and restore functional independence while collaborating with the interdisciplinary care team.

• Evaluate patients and develop individualized physical therapy treatment plans.

• Provide therapeutic exercises, mobility training, balance training, and functional rehabilitation.

• Implement treatment plans and select appropriate therapeutic modalities to meet established goals.

• Monitor patient progress and modify treatment plans as needed.

• Provide patient and caregiver education to support continued recovery and safety.

• Assist with discharge planning and recommendations for continued therapy or equipment.

• Maintain accurate and timely patient documentation in accordance with facility guidelines.

• Collaborate with physicians, nurses, and therapy team members to coordinate patient care.

• Maintain department productivity standards.

• Ensure therapy services meet age-specific, cultural, and rehabilitative needs.

• Strong written and verbal communication skills

• Ability to work effectively with interdisciplinary healthcare teams

• Ability to prioritize tasks and manage time efficiently

• Knowledge of physical therapy rehabilitation techniques and modalities

• Required: Doctor of Physical Therapy (DPT) or graduate of an accredited Physical Therapy program

• Required: Active Physical Therapist license in the state of assignment or ability to obtain

• Required: American Heart Association (AHA) BLS Certification

• Preferred: Skilled Nursing Facility (SNF) or rehabilitation experience
