Wage Range: $50-$55 DOE

Position Summary

We are seeking a PRN Physical Therapist (PT) to join our skilled nursing and rehabilitation team. The Physical Therapist is responsible for evaluating residents, developing and implementing individualized treatment plans, and providing therapeutic interventions to help residents improve mobility, strength, balance, and overall functional independence.

Job Duties & Responsibilities
  • Evaluate residents to determine physical therapy needs and functional limitations
  • Develop, implement, and modify individualized treatment plans in accordance with physician orders
  • Provide therapeutic exercises, gait training, balance training, and functional mobility interventions
  • Monitor resident progress and adjust treatment plans as appropriate
  • Accurately document evaluations, daily treatments, progress notes, and discharge summaries in a timely manner
  • Communicate effectively with residents, families, physicians, and interdisciplinary team members
  • Supervise Physical Therapy Assistants as applicable
  • Ensure compliance with all federal, state, and facility regulations and standards of practice
Qualifications
  • Current Washington state Physical Therapist license in good standing
  • Strong knowledge of physical therapy principles and skilled nursing regulations
  • Skilled nursing or post-acute care experience preferred
  • Excellent communication, time management, and documentation skills
  • Compassionate, resident-focused approach to care
