* Present a variety of State caregiving programs at various public forums;
* Complete timely and accurate Care Plans while ensuring the appropriate services are being offered to clients;
* Request authorization for client services accurately and timely;
* Facilitate caregiver support groups and educational classes;
* Promote the program by participating in outreach activities;
* Provide clients with information to assist in decision-making and problem-solving related to their caregiver roles;
* Advocate for clients when appropriate;
* Complete all documentation and entries into State databases within the required timeframe set forth by State standards;
* Maintain professional acumen when interacting with clients, staff, agency partners, and leadership;
* Requires travel between various FBH work sites on a regular or daily basis;
* Complete all other duties as assigned by the branch supervisor and/or director.
Staff administering the full TCARE
**** assessment/consultation and service planning must meet the minimum qualifications of an AAA Case Manager and certification in the State of Washington. These qualifications include the following minimum education and experience requirements:
* A Master’s degree in behavioral or health sciences and one year of paid on-the-job social service experiences;
** OR
* ** A Bachelor’s degree in behavioral or health sciences and two years of paid on-the-job social service experience;
** OR
* ** A Master’s in any other field and three years of paid on-the-job social service experience;
** OR
* ** A Bachelor’s degree in any other field and four years of paid on-the-job social service experience
* Must apply for a Counselor Agency Affiliated – Registered credential from the WA State Department of Health within 30 days of hire.
* Must have an unrestricted and valid driver’s license, active car insurance and must be willing to use their personal vehicle in the course of work.
* Knowledge of elder care and disabled adult caregiving issues;
* Ability to perform face-to-face assessments and screenings;
* Proficiency with Microsoft Office suite and experience with database systems;
* Experience with leading large groups and public speaking.
* This position provides client services in-office and within the community at large on a occasional to frequent basis.
* Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community e environments may not be ideal in terms of cleanliness or being habitable.
* Occasionally frequent travel between locations is required.
* The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.
* Daily use of a computer is required.
* May be required to lift 10 - 20 pounds.
* Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.
