Job Summary
AFamily Resources Coordinator works with families and caregivers with children ages birth to three years who have been referred due to concerns for developmental delays. This position supports families and caregivers in the coordination of services and resources throughout their time in Joya’s Early Support Services (ESS). The individual in this position has a passion for helping children and their families/caregivers.
Duties and Responsibilities:- Complete initial intakes with families and caregivers either in person, virtually or by phone as directed by Nurse Enrollment Coordinator.
- Act as a social service coordinator for families/multidisciplinary teams.
- Assist new families in applying for services such as Developmental Disabilities Administration (DDA), Supplemental Security Income (SSI), Apple Health (Medicaid) as indicated.
- Act as agency contact person for multi‑agency meetings.
- Provide in‑home, virtual or phone consultations in conjunction with Joya multidisciplinary team members.
- Develop and facilitate parent support groups and/or educational groups as agreed upon with the Program Director.
- Participate in the Medicaid Administrative Claiming (MAC) program and provide supporting documentation and timesheets for "matchable" activities.
- Act as a Family Resources Coordinator within the Early Supports for Infants and Toddlers program (ESIT).
- Collaborate with families, teachers and therapists to identify strengths and needs of the child and family in order to coordinate and develop an Individualized Family Service Plan (IFSP).
- Develop Individualized Family Service Plan (IFSP) and enter into the Early Support for Infants and Toddlers (ESIT) Data Management System (DMS).
- Coordinate transitions to school districts.
- Work with families of all socioeconomic incomes, including outreach activities and care coordination for Medicaid and Medicaid‑eligible families.
- Perform other related duties as assigned.

Required Skills & Abilities:- Thorough knowledge of community social service systems.

- Ability to maintain confidentiality of sensitive information.
- Understanding of and ability to resolve conflict.
- Knowledge of HIPAA and FERPA requirements/regulations.
- Excellent organizational skills and attention to detail.
- Proficient in Microsoft Office Suite.
Position Requirements :
- Bachelor’s degree in a health or education related field (e.g., health education, social work, public health, special education, early childhood education or nursing).
- Master’s degree in social work or RN license preferred.
- Two to three years of experience in working with families, case management, social work/social welfare, infant mental health, early childhood development or counseling preferred.
- Current registration with the WA State Early Support for Infant and Toddlers (ESIT) Early Support Services (ESS) or ability to complete the requirements within three months of employment.
- Valid driver’s license and vehicle with proof of insurance.
- First Aid / CPR Certificate.
- Bloodborne Pathogens Training (will be provided).
- T.B. Test, proof of Tdap Immunization, Hepatitis B vaccinations.
- Washington State Department of Early Learning criminal background clearance.
- DSHS Background Check clearance.
- Ability to lift or carry up to 40 pounds regularly.
- Indoors, sedentary up to 7 hours/day sitting or standing and up to 4 hours/day keyboarding.
- Ability to navigate client sites (unpaved walkways, stairs, narrow hallways, etc.) on a regular basis.
- Ability to bend, twist, stoop on a regular basis.
