Full‑time Hospice / Home Health Social Worker in Spokane, WA. Provides emotional, social, and practical support to patients and families receiving hospice or home health services. Works as key member of interdisciplinary team ensuring holistic, patient‑centered care.
Key Responsibilities:- Bachelor’s or Master’s degree in Social Work from an accredited program
- Active Idaho Social Work license
- Licensed Master Social Worker (LMSW) required
- Valid driver’s license with ability to travel to patient homes
- Strong communication, organization, and time‑management skills
- Knowledge of community resources, Medicaid, and social service systems
Skills:
- Experience in hospice, home health, medical social work, or healthcare setting
You hold an active Idaho LMSW license. You can travel to patients’ homes across Spokane. You excel at psychosocial assessments and resource coordination
