• Evaluate clients and develop individualized treatment plans based on physician's orders
• Provide in‑home physical therapy services focused on mobility, strength, and functional independence
• Educate clients and families on exercises, safety, and injury prevention
• Document evaluations, progress notes, and outcomes accurately and timely
• Collaborate with nurses, caregivers, and care coordinators to support the plan of care
• Uphold all state, federal, and agency compliance standards

• Current Washington State Physical Therapist license (required)
• Graduate of an accredited Physical Therapy program
• Previous home health or outpatient experience preferred (but not required)
• Strong communication and clinical judgement skills
• Ability to work independently while remaining connected to a team
• Reliable transportation and ability to travel to client homes
