* Job Description:
**** Purpose of Job:
** Improve the overall health of the communities we serve by providing pharmaceutical care in the following ways:
*
* Essential Duties and Responsibilities:
*** Provides drug education and counseling to patients as outlined in department policies and procedures
* Develops, documents, and executes a rational, safe, evidence-based, cost-effective patient-centered plan of care with patient’s PCP and members of the care team utilizing national guidelines or established protocol.
* Identifies patient strengths and barriers to pharmacotherapy plan and medication adherence.
* Reviews, authorizes, and changes medications for specified chronic diseases within the delegated prescriptive authority and guidelines under CHAS Health’s state approved Collaborative Drug Therapy Agreement:
* Identify and take specific corrective action for drug-induced problems
* Monitor patients for appropriateness and response to therapy
* Administer injectable medications and immunizations
* Order and analyze laboratory and diagnostic test data so as to modify drug therapy and dosing as necessary
* Coordinates with providers, registered nurses, medical assistants and pharmacy staff to prioritize patient care and provide efficient patient flow in the clinic. Utilizes registry management tools.
* Notifies primary provider of pertinent/atypical findings through documentation in the electronic health record (EHR)
* Provides effective transition of care needs including, but not limited to, medication reconciliation and completing post-discharge phone calls
* Identifies opportunities for the development of new care management approaches; takes an active role in the development and implementation of quality improvement projects
* Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.
