Location: Spokane Valley

Option Care Health, Inc. is the largest independent home and alternate site infusion services provider in the United States, with over 8,000 team members including 5,000 clinicians. The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to patients in home and alternate settings. The position is entry‑level, Monday‑Friday day shift with on‑call responsibilities.

Job Responsibilities
  • Determine the suitability of individual patients for home care. Collect and organize all patient‑specific information needed to determine suitability in accordance with Option Care policies and to set clearly defined outcome goals.
  • Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for the pharmacotherapy of home care patients. Make suitable recommendations for modifications in the prescribed medication therapy or monitoring parameters when appropriate, and complete a nutritional assessment of patients.
  • Modify pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient‑specific information; apply pharmacokinetic principles and assess patient laboratory values.
  • Provide medication‑use education to Option Care staff, other professionals, home‑care patients, and/or their caregivers, using effective counseling techniques.
  • Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings, following systematic procedures for communication and using appropriate techniques for medication preparation, labeling and dispensing.
  • Manage the use, maintenance and troubleshooting of medication administration and related equipment, including monitoring and reporting discrepancies in perpetual inventory systems for controlled substances.
  • Respond promptly and concisely to drug information requests from healthcare providers and patients, formulating responses based on literature analysis.
  • Manage investigational drug products in accordance with regulations and company policies, utilizing pharmacy support personnel effectively and ensuring accuracy of their work.
  • Comply with accreditation, legal, regulatory and safety requirements in all activities.
  • Maintain confidentiality of patient and proprietary information, observing legal and ethical guidelines.

Basic

Education and Experience Requirements
  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license granted by the State Board of Pharmacy.
  • One year of experience in at least one of the following: sterile IV admixture procedures, non‑sterile compounding and immunizations, medication therapy management, completion of an ASHP accredited Pharmacy Residency Program, or completion of an Advanced Pharmacy Practice Experience.
Basic Qualifications
  • Experience providing customer service to internal and external customers and meeting quality standards for services.
  • Basic level skill in Microsoft Word, Excel and PowerPoint (e.g., document editing, data entry, formatting and slide creation).
Travel Requirements

Willing to travel up to 5 % of the time for business purposes within the state.

Preferred Qualifications & Interests
  • Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience.
  • Experience in home care, hospital or other clean‑room setting.
Management Experience

At least one year of experience in indirect management of team members, including assisting in training and assignment of work to other team members.
