* Job Description:

**## Purpose of Job:
Improves the overall health of the communities we serve by delivering exceptional patient scheduling and supporting real‐time in-clinic care team coordination as follows:
*
* Essential Duties and Responsibilities:

*** Provides excellent customer service as a first point of contact for patients, determining caller needs, managing calls appropriately, and delivering accurate information about services, locations, providers, and access options.
* Schedules follow-up appointments before the patient leaves the clinic and, per our Team-Based Care model, provides in-room scheduling when appropriate to support continuity of care, scheduling accuracy, and enhanced patient experience.
* Conducts proactive outreach to reschedule missed appointments and actively manages a patient waitlist to improve access.
* Schedules visits in alignment with provider preferences and updated guidelines, ensuring accurate visit types, lengths, interpreter needs, and appropriate patient flow.
* Monitors provider schedules for gaps, cancellations, and optimization opportunities and collaborates with onsite care team members to fill openings and adjust schedules based on workflow needs.
* Develops and maintains expertise in clinic workflows, provider templates, and scheduling standards to support efficient operations.
* Communicates appointment details clearly and ensures patient understanding of next steps.
* Documents scheduling activity and patient communication actions in the electronic health record.
* Supports scheduling and access-related quality improvement initiatives and updates scheduling practices in alignment with new guidelines or provider changes.
* Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.#
