We provide a variety of client services, including outpatient, inpatient, community outreach, crisis services, partnerships with schools, law enforcement, and more. Our evidence‑based, culturally appropriate, trauma‑informed behavioral health services address the diverse needs of our clients. The multi‑disciplinary Homeless Outreach Stabilization Team (HOST) works in the community to provide substance abuse treatment, medical services, rehabilitative support, case management and peer support services.

The program serves homeless individuals with behavioral health challenges who have demonstrated a combination of continuous service needs and functional impairments and have not benefited from traditional outpatient programs currently available.

• Coordinate medical aspects of care, including monitoring and administration of medications in a manner that meets the unique needs of clients.
• Access the assigned nurse manager for nursing‑related consultation as needed.
• Provide education to clients regarding their treatment, medical and mental health condition.
• Develop and engage clients in health promotion activities such as health and wellness clinics, smoking cessation efforts, and client education programs for disease prevention.
• On‑going assessment of mental status and the positive impact and side effects of current treatment.
• Provide specialized training and consultation to other staff members on medical/mental health issues related to clients and their care.
• Participate in psychiatric consultation, supervision, program meetings and in‑service trainings; participate in clinical reviews and case conferences for team clients.
• Comply with the agency’s clinical accountability policies and procedures; maintain current and complete clinical records; participate in quality assurance reviews.
• Comply with applicable program research and evaluation process.
• Participate as a member of a multi‑disciplinary team providing some components of on‑going case management services to homeless and formerly homeless adults.
• Assess clients for range of health care needs and facilitate linkage and on‑going connection to relevant urgent, primary and specialty health services.
• Advocate for client’s access to community resources and services, especially primary health care services, ensuring that clients’ needs are met and rights maintained; consult and collaborate with community providers to ensure continuity of care.
• Requires travel to see clients in the community and travel between work sites either via FBH owned vehicles and/or personal vehicle.
• Other duties as assigned.

• RN licensed in the State of Washington.
• One year experience in the direct treatment of individuals with mental illness.
• Active BLS within 90 days of hire and must maintain active BLS.
• Unrestricted and valid driver’s license, pass a motor vehicle screening and driving test, have active car insurance, and be willing to use personal vehicle in the course of work.
