** Purpose of Job:
** Improve the overall health of the communities we serve by providing individualized nutritional support to patients for their particular situation as follows:##
Essential Duties and Responsibilities:
* Meets directly with patient to identify dietary needs through nutritional assessment based on nutrition-focused physical findings and patient history.
* Provides nutritional counseling either individually or in a group setting; identifying goals and supporting education with evidence-based care.
* Develops detailed care plans and documents as part of the electronic health record in accordance with established program standards.
* Assesses, plans and implements the delivery of medical nutrition therapies in collaboration with the patients inter-disciplinary team.
* Monitors and evaluates patient's progress through routine nutrition interventions to support positive health outcomes.
* Participates and supports patients in various program committees; including developing educational programs for various ethnic, cultural and social groups to teach nutrition education.
* Remains up to date on evidence-based research and recommends ways to expand and improve nutrition programs.
* Participates in quality improvement of the program in conjunction with other services CHAS Health offers.
* Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.##
Qualifications:
Education/
Experience:
Bachelor’s degree in Nutrition or related field required. Registered Dietitian credentialed with the Commission on Dietetic Registration and eligible for state licensure. Experience in chronic disease including HIV and Diabetes and/or maternity nutrition counseling preferred.
Skills:
