Home Health Physical Therapist (PT) – Travel/Contract
Start Date:

We are seeking a dedicated and skilled Home Health Physical Therapist (PT) for an immediate opportunity with a respected home health provider in Washington State. This position offers an ideal schedule, supportive systems, and flexible patient scheduling, making it a great fit for both experienced travelers and local clinicians. Position Highlights:
Caseload:
Adult patients only
Daily Visits: 4–5 per day
Mileage Reimbursement: IRS standard rate
Scrubs:
Any color permitted
Charting System:
Home Care Home Base (HCHB) – Required Documentation Equipment:
Provided (must be returned)
Driving Radius:
Occasionally up to 100 miles, though routes are grouped efficiently to minimize windshield time
Transportation:
Must have own reliable transportation
Requirements Required Experience &
Qualifications:
Minimum 1 year of recent Skilled Home Health PT experience (within the last 3 years)
Inpatient-only experience will not be accepted
Current Washington PT License
BCLS Certification (Basic Cardiac Life Support)
Strong understanding of OASIS documentation
Must be comfortable working independently in home settings
