LICSW / LSWAIC Social Worker
Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.
Minimum Required Qualifications:
- WA LICSW or LSWAIC (only if they do not need supervision)
- MSW degree
- 2 years acute care hospital
* or* 1 year acute plus 1 year of Hospice, HH or other work with elderly - EPIC experience
- Discharge planning experience
- Experienced in Drug & Alcohol Screens (SBIRT), Mental Health Evals (SAFE-T), Capacity Evals, Behavior Plans, BH referrals, Provide or Assist with Tele Psych Evals, end of life conversations, Single Bed Certs/ITA process, difficult placements, prior experience in home health, hospice, SNF's, Medicaid/Medicare, insurance authorizations.
Length of Assignment: 13 weeks
Shift / Hours Per Week:
Nights. Option for 10s or 12s typically.
