Hospice And Home‑Health Social Worker
This full‑time hospice and home‑health social worker in Spokane, WA supports patients and families through emotional, social, and practical care, coordinating community resources and ensuring holistic, patient‑centered care. The role is a key member of the interdisciplinary team, advocating for patient rights and quality of life.
Key Responsibilities:- Conduct psychosocial assessments for hospice and home health patients
- Provide emotional support, counseling, and crisis intervention for patients and families
- Assist patients and caregivers with advance care planning and end‑of‑life support
- Connect patients with community resources and support services, including Medicaid application assistance, Meals on Wheels, medical transportation, caregiver support, utility assistance, and housing assistance
- Coordinate respite care services and hospice transfers
- Collaborate with nurses, physicians, chaplains, and interdisciplinary team members
- Advocate for patient rights, dignity, and quality of life
- Maintain accurate documentation in compliance with hospice and home health regulations
- Bachelor's or Master's degree in Social Work from an accredited program
- Active Idaho Social Work license
- Licensed Master Social Worker (LMSW) required
- Knowledge of community resources, Medicaid, and social service systems
- Strong communication, organization, and time‑management skills
- Valid driver's license with ability to travel to patient homes
Skills:
- Experience in hospice, home health, medical social work, or healthcare setting preferred
