ADULT OUTPATIENT Our Adult Outpatient Services uses a person-centered model of care to assist individuals 18 or older who are experiencing mental health issues. We strive to pair each individual's treatment objectives with evidence-based clinical practices in a practical fashion. A high priority is placed on developing therapeutic relationships that promote resiliency and recovery.

Services include:

Individual Therapy Group Therapy Psycho-educational Classes Special population consultations and evaluations

Case Management Psychiatric medication management

Peer Support Specialists In this position, you will provide trauma-informed, person-centered, evidence-based therapy to clients served within an FBH outpatient program. Participate in a multidisciplinary team by providing therapeutic interventions, writing and implement recovery care plans, provide client education, advocate on behalf of clients and conduct risk assessments while measuring client progress toward established goals and objectives in virtual, office, or community-based settings.

ESSENTIAL DUTIES & RESPONSIBILITIES:

1. Provide episodic, trauma-informed, person-centered therapy utilizing evidence-based practices.
2. Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices.
3. When required by the outpatient program, MHC IIs who hold a credential (i.e., L-AAC, LMHCA, LMFTA, LICSWA) eligible to be a Mental Health Professionals will provide initial intake assessments for persons seeking services at FBH to determine individual meets medical necessity and eligibility for FBH outpatient services.

Involves comprehensive assessments, preliminary diagnoses, and treatment recommendations for both youth and adults.

Involves completing Consent to Release Confidential Information with the client and guardians, when appropriate, for any current and past behavioral health treatment, involved allied service provider, and natural supports.

Intakes should be completed concurrently with the client.

Accurately determine the program assignment for individuals who are enrolled.
4. Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
5. Facilitate and co-facilitate client treatment groups.
6. Represent the agency and its services to the community in a positive and professional manner.
7. Participate in required staff meetings, including regular supervision and weekly clinical staff meetings.
8. Participate in any required training, consultation calls, and onsite visits or reviews when required by contract.
