Direct Support Professional

As a Direct Support Professional at Sevita, you'll provide person-centered care to individuals with disabilities and complex needs in home and community settings. You'll assist with daily living, personal care, safety, and community integration while supporting behavioral health and rehabilitation goals. Working closely with a collaborative team, you'll document progress, follow individualized care plans, and respond to emergencies. Responsibilities

• Provide person-centered support to individuals with disabilities in home and community settings
• Assist with daily living activities, including personal care, hygiene, and meal preparation
• Support behavioral health and brain injury rehabilitation goals as part of an interdisciplinary team
• Implement individualized service plans and document progress accurately and timely
• Ensure client safety, follow infection control standards, and respond appropriately in emergencies
• Offer transportation and community integration support to promote independence
• Communicate effectively with individuals, families, and supervisors to coordinate care
• Participate in ongoing training and professional development to maintain high-quality care
