Description:
The Health Information Management Clerk is responsible for the day-to-day operation of scanning, examining, faxing, and processing all patient records. Answering all incoming medical record calls and handling all release of information requests (ROI). Process all workers compensation documentation chart notes and mailing them and/or uploading to Aspirion. Assist staff and physicians with questions relating to the medical record and maintain strict confidentiality at all times regarding patient information.
Minimum qualifications:
High school diploma or GED equivalent preferred; college courses in Health Information or Medical Office preferred.
Medical Records or physician office medical record clerk experience preferred. Comfortable with computer applications.
Computers, 10-key, copy machine, fax machine, staplers, and hole punchers.
Primary job responsibilities:
The Health Information Management Clerk acts as a primary contact person in the HIM department. Answers incoming phone calls and directs as appropriate. Assists other hospital personnel and medical staff that come to the HIM department.
Responsible for release of information. Verify legality of requests for health information while conforming with all applicable state, federal and hospital requirements. Maintains strict confidentiality of all patient records and information.
Performs discharge analysis on all charts to ensure presence of required reports and provider signatures. Incomplete records are routed to appropriate personnel for completion. Notifies providers of incomplete charts.
Scans all discharged charts per chart order, all loose sheets, documentation from outside facilities into the computer per department protocol.
Assembles all discharged charts. This includes ER, Short Stay, Surgical, Inpatient, and Skilled Swing Bed.
Provides Administrative assistance as necessary to Revenue Cycle Director and other tasks as assigned.
Update charges as needed, new addresses, name changes, marking deceased, phone numbers, etc.
Completes workers compensation form and sends to appropriate agency.
Knowledgeable in the use of all office equipment and computers in HIM department.
Able to work under stress, maintaining professional demeanor.
Merge duplicate medical records.
Process all DME required information.
Cover for Health Information Analyst as needed.
Pull medical records for billers to resolve unpaid claims.
Handle all incoming and outgoing cd's for imaging.
