Center Well Home Health is seeking a Home Health Occupational Therapist Assistant to support patients in regaining strength, mobility and independence in their homes. You will work under the supervision of an Occupational Therapist, perform therapy, adapt equipment, and participate in care coordination with the interdisciplinary team to promote functional recovery and safe discharge.
Location:
travel to patient homes within the service area, with base at the Home Health Branch Administration.
