Elder & Adult Services provides in‑home and agency clinical assessments and services to clients (Medicaid/Non‑Medicaid) 21years of age or older. Elder & Adult Services utilizes the Gatekeeper Case Finding Model to proactively locate, identify, and serve clients in need of support and assistance in their home. Elder Services serves as the primary access point for services and resources that promote independence and enhanced quality of life for vulnerable older adults who are at risk or isolated.

In this position , you will provide trauma‑informed, person‑centered, evidence‑based therapy to clients served within an FBH outpatient program. Participate in a multidisciplinary team by providing therapeutic interventions, writing and implement recovery care plans, provide client education, advocate on behalf of clients and conduct risk assessments while measuring client progress toward established goals and objectives in virtual, office, or community‑based settings.

• 1. Provide episodic, trauma‐informed, person‑centered therapy utilizing evidence‐based practices.
• 2. Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices.
• 3. When required by the outpatient program, MHC IIs who hold a credential (i.e., L-AAC, LMHCA, LMFTA, LICSWA) eligible to be a Mental Health Professionals will provide initial intake assessments for persons seeking services at FBH to determine individual meets medical necessity and eligibility for FBH outpatient services. Involves comprehensive assessments, preliminary diagnoses, and treatment recommendations for both youth and adults.
  
  Involves completing Consent to Release Confidential Information with the client and guardians, when appropriate, for any current and past behavioral health treatment, involved allied service provider, and natural supports. Intakes should be completed concurrently with the client. Accurately determine the program assignment for individuals who are enrolled.
• 4. Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
