In-Home Health NP — Part-Time Clinician

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for members in Washington. Clinicians will visit homes, review medical histories, conduct physicals, and coordinate care with health plans.

You’ll travel locally and may conduct virtual visits as needed, with about 29.5 hours per week. Board-certified NP/PA licenses in WA are required; multi-state licenses preferred.
