Full-time hospice/home health social worker based in Spokane, WA. Provide emotional, social, and practical support to patients and families, ensuring holistic, patient-centered care throughout their journey.
Key Responsibilities:Conduct psychosocial assessments for hospice and home health patients
Provide emotional support, counseling, and crisis intervention for patients and families
Assist patients and caregivers with advance care planning and end-of-life support
Connect patients with community resources, including Medicaid assistance, nutrition programs, transportation, caregiver support, utility assistance, and housing services
Coordinate respite care services and hospice transfers as needed
Collaborate with nurses, physicians, chaplains, and interdisciplinary team members
Advocate for patient rights, dignity, and quality of life
Maintain accurate documentation in compliance with hospice and home health regulations
Bachelor's or Master's degree in Social Work from an accredited program
Active Idaho Social Work license
Licensed Master Social Worker (LMSW) required
Knowledge of community resources, Medicaid, and social service systems
Strong communication, organization, and time-management skills
Valid driver's license with the ability to travel to patient homes
Skills:
Experience in hospice, home health, medical social work, or healthcare setting
You hold an active LMSW license in Idaho. You excel at psychosocial assessments and end-of-life counseling. You can travel to homes and coordinate community resources
