As a Hybrid Clinic Triage Nurse
, you will provide specialized nursing care in an ambulatory cardiology setting, performing telephone triage, patient education, lab result reviews, and assisting with in-clinic procedures, including IV Lasix administration. This role is essential in ensuring efficient and compassionate care for heart failure patients.
- Conduct nurse telephone triage
, providing clinical guidance to patients. - Educate patients and families on heart failure management, medications, and disease progression.
- Manage and administer IV Lasix therapy in the clinic.
- Review lab results and follow up with patients as needed.
- Coordinate care and referrals to other specialties, including general cardiology, electrophysiology, and community resources.
- Assist in emergency situations within the clinic.
- Collaborate closely with medical assistants, schedulers, and providers to support patient care.
- Maintain compliance with HIPAA and other healthcare regulations.
Qualifications
- Current Washington State RN license required
- Bachelor’s degree in Nursing (BSN) preferred; associate degree considered with additional experience
- Master’s degree in Nursing is a plus
- 3–5 years of clinical experience
, preferably with cardiac or heart failure patients - Knowledge of GDMT and heart failure case management
- Strong interpersonal, communication, and critical thinking skills
- Passion for patient-centered care and health education
- Location: DMOB with frequent trips to DHEC
- Schedule: Flexible schedule TBD
- Environment: In-office, clinical setting with regular walking between hospital floors and clinic locations
- Travel: Local, between partnered locations only
