Position: Restorative Nurse (LPN or RN)

A Restorative Nurse job description involves implementing individualized care plans to help patients regain/maintain function, focusing on mobility (ROM, ambulation), self-care (dressing, eating, toileting), and preventing decline through specific exercises, therapy-directed activities, training staff, and detailed documentation, often under an RN/LPN supervisor. Key duties include documenting progress, monitoring assistive devices, coordinating care, and training CNAs to foster independence and improve quality of life in long-term care settings.

Key Responsibilities
  • Care Implementation:
    Carry out prescribed restorative/rehabilitative care plans from PT/OT/Nurses.
  • Mobility Training:
    Assist with active/passive range of motion (ROM), strengthening exercises, functional ambulation, and proper turning/positioning.
  • Self-Care Training:
    Help residents with dressing, grooming, eating, swallowing, and using assistive devices.
  • Bowel/Bladder Programs:
    Manage continence programs and bladder retraining.
  • Documentation:
    Chart progress, responses, and report changes to the nursing team and therapists.
  • Staff Training:
    Educate CNAs on restorative techniques and care plan implementation.
  • Coordination:
    Work with therapists, nurses, and other departments (e.g., Recreation) to integrate care.
  • Monitoring:
    Oversee the use of splints, orthotics, and other devices.
Skills & Qualifications
  • Current RN or LPN license (for supervisory roles).
  • Specialized training in restorative care (often includes CNA certification with specialty).
  • Strong communication and documentation skills.
  • Ability to work with diverse populations and promote resident rights.
  • Understanding of rehabilitation principles, fall prevention, and cognitive/physical needs.

  • Increase resident independence and self-esteem.
  • Prevent secondary complications (e.g., contractures, pressure sores).
  • Minimize social isolation and depression.
  • Help residents maintain or regain functional abilities within their limitations.
