Full‑time Clinician (Nurse Practitioner or Physician Assistant)

Signify Health, part of CVS Health®, is seeking a full‑time clinician to provide in‑home health evaluations in the comfort of patients’ homes. The role involves building trusted relationships and delivering comprehensive, personalized health assessments, including medication and medical history review, physical evaluation, and various diagnostic tests when ordered by the health plan.

• Provide in‑home health evaluations lasting approximately 45 minutes, connecting one‑on‑one with patients and answering health questions.
• Conduct medication and medical history reviews, physical examinations, and additional diagnostics such as diabetic eye exams, spirometry, and other preventive services as indicated.
• Use an iPad and an intuitive clinical workflow for each evaluation; do not prescribe medicine, order lab tests, or alter existing treatment regimens.
• Work flexibly to travel locally and within licensed states, and conduct virtual visits when needed.
• Collaborate with Signify Health as a clinician colleague, fulfilling the role’s requirements and providing all diagnostic and preventive services products, including spirometry, DEE, etc.

As an in‑home clinician, the primary workplace is the private residence of members. Candidates must be able to perform duties safely in environments that may contain household pets, secondhand smoke, or airborne allergens.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required). Multi‑state licenses (preferred).
• Ability to cover primary locations:
  Kennewick/Benton County.
• Eligible for employment regardless of arrest or conviction records, in accordance with federal, state, and local laws.

Pay Range: $95,738.00 – $ annually, based on experience, education, geography, and other factors.
