In this role, you’ll provide direct patient care in homes, adult family homes, and facilities, supporting patients and families through end‑of‑life care with comfort, dignity, and compassion. What You’ll Do
  • Travel to multiple patient homes each day, providing 4–6 visits on average.
  • Assist patients with daily living activities (bathing, grooming, toileting, dressing).
  • Monitor and report vital signs and patient conditions.
  • Administer medications under nurse supervision.
  • Offer emotional support and companionship to patients and families.
  • Maintain a safe, clean, and dignified care environment.
  • Collaborate with the interdisciplinary care team on individualized care plans.
  • Document patient care accurately and comply with HIPAA regulations.
Qualifications
  • Certified Nursing Assistant (CNA) licensed in Washington State.
  • Experience in home health or hospice agency highly preferred.
  • Compassionate, patient‑focused, and empathetic.
  • Reliable, insured transportation and valid WA driver’s license.
