Overview

Providence at Home with Compassus is a collective bargaining position for a Home Health Registered Nurse (RN). Day shift, 4 days/flexible – 32 hours, full time.

We’ve brought together two industry leaders to deliver exceptional home health and hospice services. We’re currently seeking a dedicated and experienced Home Health Registered Nurse (RN) to provide expert care in patients’ homes, where healing happens best. The RN will work independently and as part of an interdisciplinary team to conduct comprehensive assessments, develop and implement individualized care plans, and deliver outcome-driven interventions under the physician-ordered Plan of Care.

This role emphasizes community health nursing and patient- and family-centered care, following the nursing process in accordance with the Nurse Practice Act (assessment, planning, implementation, evaluation).

Position Specific Responsibilities
  • Possesses knowledge of developmental needs in the assessment and treatment as it relates to the age of the patient population served.
  • Conducts and documents a comprehensive patient assessment upon admission.
  • Continually reassesses and applies appropriate advocacy and interventions as indicated.
  • Interprets a patient’s self-assessment and behavior with consideration of cognitive, physical, emotional, psychosocial, and maturation factors.
  • Collaborates with the patient, family and healthcare team; develops, documents, implements and monitors an individualized plan of care reflecting best practice and evidence-based medicine.
  • Adapts to change and updated patient care practices within an evolving healthcare environment.
  • Legibly and electronically documents patient care activities in an accurate, concise, and timely manner.
  • Supports and assists with orientation of new staff and students.
  • Acquires necessary training and skills to competently perform preceptor and charge nurse functions when selected for those roles.
  • Demonstrates advancement of professional nursing practice through continuing education, required training, certification, and participation in department activities.
  • Takes an active role in building and maintaining an environment that fosters open communication, patient- and family-centered care, and healthy collegial relationships.
  • Demonstrates adaptive/timely communication based on individual patient/family needs.
  • Attends and participates in scheduled staff meetings or reviews minutes as required.
Education and/or Experience
  • Required — Associate or Bachelor's degree from an accredited School of Nursing.
  • Preferred — 1 year recent experience in home health, acute care or a skilled nursing facility.
Certifications, Licenses, and Registrations

Required — Current, unencumbered (State of Employment) License as a Registered Nurse.

Required — National Provider BLS - American Heart Association (Vendor Managed).

Physical Demands And Work Environment

The demands of this role require a team member to perform essential functions. Adaptations can be made for disabilities. Regular standing, walking, and manual dexterity are required, with the ability to lift and move objects up to 50 pounds. Visual acuity includes close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Exposure to bodily fluids, infectious diseases, and field conditions is expected.

Routine use of standard medical equipment and tools is essential. Notes may vary by role and department.

Compensation and Benefits

Compensation range: $40.04 - $72.09 per hour. The base pay range listed; additional compensation may be available (shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities).
