Shift Type: Rotating day shifts, 7 on / 7 off (working Friday through Thursday during on weeks), 12:00 PM - 10:30 PM (Day shift)
Hours per Week: 40
Candidate Pay Rate: $69 per hour

- Perform admissions, triage, and intake assessments for hospice-eligible patients with professionalism and compassion
- Conduct skilled RN visits in patients’ homes, delivering symptom assessment and management
- Administer medications safely and manage pain control plans including breakthrough dosing
- Provide end-of-life care and family support, including death pronouncement and perideath counseling
- Document thoroughly and accurately in Epic and Home Care Home Base to ensure continuity of care
- Educate patients and families on care goals, medication use, positioning and preventive measures
- Coordinate with interdisciplinary team members (social services, pharmacy, interpreters) to optimize patient-centered plans
- Help prioritize visits and routes to reduce drive time while maintaining high-quality patient contact
- Active RN license (pending license accepted as noted) — must be eligible to practice in WA
- Minimum 1 year of skilled hospice nursing experience required
- Home Care Home Base (HCHB) experience required;
Epic charting experience preferred - BLS certification required
- Reliable personal transportation (traveler expected to have own vehicle); able to travel occasional 50+ mile radius
- Comfort with admissions, triage, medication administration, symptom assessment and end-of-life care
- Not accepting first-time hospice RNs
- Patient admissions, triage and skilled RN visits
- Charting in Epic and Home Care Home Base
- Medication administration and pain management (pharmacologic and non-pharmacologic)
- Symptom assessment and management including pressure ulcer staging and altered mental status
- End-of-life care, death pronouncement and family counseling
- Knowledge of Medicare state regulations and DME authorization necessity
- Cultural awareness and patient/family education
- Goal setting with patient/family and intake assessment/interview skills
