JOB SUMMARY

The registered nurse plans, organizes, and directs home health care and is experienced in nursing with an emphasis on community health education. The professional nurse builds on community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.

DUTIES & RESPONSIBILITIES
  • Patient Care
    • Complete initial, comprehensive, and ongoing assessments of patients and families to determine home health needs.
    • Provide a complete physical assessment and history of current and previous illnesses.
    • Utilize all elements of the nursing process to provide professional nursing care.
    • Write and initiate a plan of care; regularly re‑evaluate patient and family/caregiver needs and revise the plan as necessary.
    • Use assessment data to determine nursing diagnosis and develop a care plan with goals based on diagnoses, incorporating rehabilitative nursing actions.
    • Include the patient and family in the planning process; initiate preventive and rehabilitative procedures.
    • Administer medications and treatments as prescribed by the physician; counsel the patient and family in meeting nursing and related needs.
    • Provide health‑care instructions per assessment and plan; assist with activities of daily living and facilitate self‑sufficiency.
  • Communication
    • Complete, maintain, and submit accurate clinical notes regarding patient condition and care.
    • Record pain/symptom management changes and outcomes as appropriate.
    • Communicate with the physician about patient needs, report changes, and obtain orders.
    • Coordinate with community health personnel to align the care plan.
    • Teach patients and families self‑care techniques, medication, diet, and other instructions.
    • Work in concert with the interdisciplinary team and maintain a safe environment.
    • Assist the patient, family, and team members in providing continuity of care and meet emotional and physical needs.
    • Attend case conference meetings.
  • Additional Duties
    • Participate in on‑call duties as defined by policy.
    • Ensure arrangements for equipment and necessary services are available.
    • Supervise ancillary personnel and delegate responsibilities when required.
    • Maintain professional knowledge and practice skills through continuing education and in‑service classes.
    • Participate in quality assessment, performance improvement teams, and activities.
    • Perform other duties as assigned.
JOB REQUIREMENTS
  • Graduate of a National League for Nursing accredited school of nursing.
  • Minimum of two years of experience, at least one year in public health, home care, or home health nursing.
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Demonstrate excellent observation, problem‑solving, verbal and written communication skills.
  • Show ability to organize and prioritize workload independently.
  • Physical requirements:

    prolonged or considerable walking or standing; ability to lift, position, and/or transfer patients; lift supplies and equipment; reach, stoop, bend, kneel, and/or crouch.
  • Visual acuity and hearing sufficient for required nursing skills.
  • Maintain a current CPR certification.
  • Drive or have access to transportation for agency‑related travel; valid driver’s license and automobile insurance.
  • Self‑directed and able to work with minimal supervision.
  • Responsible for supervising home health aides.

COMPENSATION

Salary: $55 per hour.
