Providence offers up to $15,000 hiring bonus for eligible external candidates who meet all conditions for payment.
The External Case Manager performs activities and duties related to preadmission evaluation and admission of patients to services along the rehabilitation continuum of care. The role also involves relationship building for marketing, patient census building, and coordination of care in collaboration with acute care case managers, social workers, and discharge planners to facilitate a smooth transition and the most appropriate level of care.
The External Case Manager works with appropriate funding sources to establish the most effective manner of managing the human, material, and financial resources available for the patient’s care.

Required Qualifications- Upon hire:
Washington Registered Nurse License. - 3 years clinical experience.
- Bachelor’s Degree in Nursing.
- Upon hire: CRRN and/or CCM certifications.
- Experience in case management; clinical nursing or utilization management.
- Experience in the rehabilitation of chronically or catastrophically ill or injured individuals.
$39.32 - $68.95 per hour. Additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
