Home Health & Hospice Assistant Clinical Director

The Assistant Clinical Director leads clinical operations and provides patient care in home health and hospice settings. The role blends direct patient care with leadership responsibilities to ensure high‑quality care, regulatory compliance, and staff support.

• Provide direct patient care in home health and hospice settings

• Oversee daily clinical operations and workflow coordination

• Mentor, educate, and guide clinical staff

• Collaborate with interdisciplinary teams to ensure quality outcomes

• Ensure compliance with state, federal, and accreditation standards

• Participate in care planning, case reviews, and quality improvement

• Monitor clinical documentation for accuracy and regulatory compliance

• Support staff scheduling, patient assignments, and operational needs

• Implement policies and procedures that promote patient‑centered care

• Serve as a clinical resource for nursing and support staff

• Active Registered Nurse (RN) license

• BLS certification

• Previous leadership or supervisory experience

• Strong communication, organizational, and leadership skills

• Ability to balance clinical responsibilities with administrative leadership duties

• Bachelor of Science in Nursing (BSN)

• Home health and/or hospice experience

• Knowledge of home health and hospice regulations and standards
