The Registered Nurse Care Manager establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination, and monitoring and ongoing routine evaluation that ensures medically and financially appropriate care.

Responsibilities include:

facilitating interdisciplinary team care coordination, intensive medical management of participants.

• Education sufficient for clinical licensure.
• On hire:
  Washington Registered Nurse License
• On hire:
  National Provider BLS - American Heart Association
• 1 year Long term care or geriatric nursing.

• Bachelor's Degree in Nursing
• Medical surgical or rehabilitation medicine.

The wage range for this position is posted as $44.61 - $69.26 hourly. The base pay range is the starting point based on experience and applicable factors. Additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
