Position: Prenatal Nurse - New Britain - Full Time - $1,000 Signon

Job Description

Full time opportunity, with one Saturday per month.

The Primary Care Prenatal Nurse is a registered nurse who specializes in providing high‑quality comprehensive care to childbearing individuals, pregnant and postpartum persons, and their families. The nurse works at the top of licensure to enhance safety, effectiveness, and efficiency of care, delegating technical and clerical functions as appropriate to medical assistants. The Prenatal Nurse supports providers, safeguards patients, improves clinical outcomes, coordinates care, serves as teacher, advocate, and navigator, and collaborates with providers for transitions of care, chronic care management, and rapid cycle improvement.

Role

and Responsibilities
  • Support primary care of patients under the planned care model, including pre‑visit planning, daily huddles, and nurse‑led and nurse‑assisted visits.
  • Triage patients both in person and over the phone based on clear triage protocols.
  • Collaborate with PCPs to identify needs for preventive care, evidence‑based chronic disease care, and care coordination.
  • Provide general and specific health education during visits and by telephone.
  • Work with patients to identify and monitor self‑management goals.
  • Collaborate and coordinate care with community‑based providers and attend Continuation of Care Collaboration meetings with local hospital child birthing centers.
  • Follow up on ER visits and hospital discharges, ensuring OB discharge follow‑up occurs.
  • Assist providers with exams, treatments, and procedures as needed.
  • Perform placement and monitoring of patients for prenatal NSTs.
  • Ensure safe, accurate, clinically appropriate collection, storage, and processing of all biologic specimens.
  • Administer medications, including immunizations, at provider order and standing orders; adhere to CHCI protocols for documentation.
  • Monitor patient response to medication, detect and report adverse reactions, reconcile medications, and provide education.
  • Perform nursing activities such as wound care, HTN follow‑up, Rhogam administration, gestational diabetes education, etc.
  • Conduct standing‑order nursing visits (prenatal intakes, uncomplicated UTIs, insulin titration, treatment of vulvovaginal candidiasis, pregnancy testing, etc.).
  • Accurately document all aspects of patient care in the medical chart on a timely basis.
  • Maintain strict infection control standards per CHCI protocols and universal precautions (JC, OSHA, CLIA).
  • Serve as a resource for medical assistants on patient preparation and procedural needs.
  • Monitor and follow up abnormal cancer screening results, STIs, and other tests performed at CHCI.
  • Oversee maintenance of supplies and equipment; order supplies as necessary.
  • Maintain compliance with mandatory annual training (Patient Safety, Abuse, Age‑Based Competencies, EOC, Abuse trainings, etc.).
  • Expertly use the electronic health record for timely, safe, confidential storage and retrieval.
  • Participate in Project ECHO complex care management to advance knowledge and skills in care management and coordination.
  • Present nursing information and insights at Integrated Care Team meetings.
Qualifications
  • Preferred:
    Bachelor of Science in Nursing (BSN).
  • Experience in a prenatal or maternal health care setting.
  • Knowledge of current prenatal care guidelines and practices.
