As part of Optum Home & Community Care's House Calls program, you will conduct annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, preventive care role focused on identifying health needs, closing gaps in care, and connecting members to appropriate resources.
Key Responsibilities- Conduct comprehensive in-home health assessments
- Review medical history, medications, vital signs, and perform physical exams
- Complete screenings and point-of-care testing as appropriate
- Document diagnoses and support care management planning
- Communicate findings to Primary Care Providers (PCPs)
- Educate patients on conditions, medications, and treatment compliance
- Address social determinants of health and coordinate referrals
- Identify urgent or emergent health concerns and respond appropriately
- ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care)
- NCCPA certification
- Active, unrestricted state license (or ability to obtain)
