Job Overview

The PRIME Case Worker provides strengths‑based, client‑centered case management to vulnerable refugee households, including survivors of domestic violence, older refugees, youth without guardians, refugees with mental health or physical disabilities, and LGBTQ clients.

Major Responsibilities
  • Collaborate closely with the ICM team to identify additional healthcare needs for vulnerable clients.
  • Attend team meetings and partner meetings as assigned.
  • Assist clients in navigating healthcare systems, public benefits, educational institutions, and other public service providers – scheduling and attending appointments when required.
  • Develop and distribute self‑sufficiency guides tailored to each client based upon self‑sufficiency goals.
  • Assist clients in selecting public and community resources that may bridge self‑sufficiency gaps and help schedule initial appointments; ensure clients are comfortable contacting providers and scheduling their own appointments.
  • Conduct regular outreach to healthcare providers and social service agencies to address barriers to services and identify resources.
  • Provide clients with extended orientation on a variety of health topics such as women’s health, specialized treatment for people with physical disabilities, navigating physical healthcare systems at the local level, public benefits, and housing; support clients in practicing and learning to do this independently.
  • Respond to client stability crises with guidance from supervisor as needed.
  • Assist in developing custom Self‑Sufficiency plans and provide coaching and support to clients as they work to remove barriers to gain self‑sufficiency.
  • Maintain up‑to‑date documentation in client case files; document all services in IRC’s database, Efforts to Outcomes (ETO).
  • Attend relevant training courses and meetings at the local and national level.
  • Train and lead the activities of volunteers and interns as required.
  • Follow all policies, procedures, and protocols of the agency.
  • Attend and engage in all team meetings, staff meetings, and other relevant meetings; participate fully as a member of the IRC team.
  • Attend trainings required of PRIME staff and implement content in work with clients.
  • Perform other duties as assigned.
Key Working Relationships

Reports to:

Health and Wellness Program Manager.
Supervises:
Interns and volunteers related to cases.
Internal contacts: other IRC Spokane and Seattle program teams, IRC Spokane Finance and Operations teams, HQ ICM.
External contacts:
Health service providers, social service providers.

Job Requirements
  • Relevant degree or equivalent professional and/or lived experience in health access, health navigation, public health, social work, social service provision, or intensive case management.
  • Lived experience as a refugee or immigrant preferred; in lieu of lived experience, experience working with refugees and/or immigrants or cross‑cultural or overseas experience accepted.
  • Minimum of 3–5 years of case management experience with similar populations and/or client needs.
  • Strong relationship‑building, diplomacy, and networking skills; ability to effectively build internal and external relationships.
  • Demonstrated success working and communicating effectively in a multicultural environment.
  • Self‑starter with excellent problem‑solving skills and the proven ability to multi‑task, prioritize duties, and manage time effectively.
  • Attention to detail and accuracy in work product.
  • Professional proficiency in English, both spoken and written; proficiency in Dari and/or Pashto strongly preferred.
  • Proficient in Microsoft Office applications (Word, Excel, Outlook); ability to use the internet and other digital tools for data entry, research, and problem‑solving.
  • Driver’s license, reliable transportation, and appropriate insurance.
  • Ability to work remotely and access a reliable Internet connection.
  • Commitment to anti‑racism and ongoing learning.
Compensation

Pay rate $25.50 – $27.50 per hour, based on location, individual experience, and skills relative to defined job requirements.
