Social Worker - Adolescent Partial Hospitalization Program

Job Summary

The Adolescent Partial Hospitalization Program (PHP) is seeking a licensed social worker to co‑facilitate and facilitate therapeutic groups and provide direct support to adolescents ages 13–18 during program hours (9:00 a.m.–3:00 p.m.).

Location:

Onalaska, WI – Onsite

Schedule:

40 hours/week, Monday – Friday, 9:00 a.m.–3:00 p.m.

Job Type: Full‑time

The Adolescent PHP Social Worker functions as an essential part of the treatment team; coordinating patient care, providing or assisting with crisis intervention, meeting with patients, facilitating/co‑facilitating groups, and assisting with programming needs. This individual will have strong communication skills, be task driven, enjoy working with adolescents, and have an appreciation for a team approach. Act as a consultant and leader in comprehensive care throughout the system, while supporting organizational strategies and goals.

• Bachelor’s degree in Social Work or in a related field
• WI Social Worker license
• 1 year of experience in human services or health care
