We are seeking dedicated HHAs and CNAs to provide 1-on-1 care in home settings, including clients served through the APD program. In these roles, care goes beyond basic assistance—you’ll play a key part in supporting independence, maintaining routines, and building meaningful relationships with clients.

• Working with APD clients focuses on:
• Supporting independence and daily living skills
• Following structured routines and consistent schedules
• Providing engagement, companionship, and encouragement
• Offering patient, reliable support in a home environment

• Provide personal care and assistance with daily living activities
• Support client safety and follow established care plans
• Encourage independence and participation in daily routines
• Assist with mobility, hygiene, and companionship
• Observe and report any changes in client condition
• Communicate effectively with the care team and family
• Maintain accurate and timely documentation
