, is seeking a compassionate PRN Home Health OT Occupational Therapist to work with us to provide exceptional and individualized care in Hernando county (Brooksville, Spring Hill, Weeki Wachee, and surrounding areas).

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.
Locations: Home health visits in Hernando county (Brooksville, Spring Hill, Weeki Wachee), and surrounding areas. If desired, visits may also be available in Pasco or Hillsborough counties.
Schedule: PRN; 5-15+ home health visits per week.
What You'll Do- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled occupational therapy services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care
- Flexible scheduling and manageable caseloads

- Active Florida OT license
- Minimum of 1 year experience as a licensed Occupational Therapist (OT).
- Home health care experience preferred.
- Strong assessment, critical thinking, and patient education skills
