We are looking for an LPN to join our Suncrest Home Health team.

• Provide quality patient care and perform skilled nursing care in the patient’s home.
• Provide services within the scope of practice as defined by state laws governing nursing practice, in accordance with the plan of care.
• Instruct the patient’s family in the care of the patient and maintain a healthy environment for the patient.
• Observe, record, and report to the supervising nurse and/or physician the patient’s response to treatment and changes in the patient’s condition daily.
• Collaborate closely with physicians and RNs, and document care accurately and promptly, without altering the plan of care.

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and reliable vehicle or public transportation access.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Work Location:

On the road – travel to multiple patient residences in the assigned geographic area.
