Position Summary

The Clinical Team Lead is responsible for ensuring that patient care is coordinated and managed appropriately, for delivering services as ordered by physicians, and for supervising clinical personnel.

Essential Responsibilities and Duties
  • Develop and initiate the plan of care, overseeing interdisciplinary assessment and ongoing development of individualized care in partnership with the patient, family, and caregivers.
  • Initiate preventive and rehabilitative nursing procedures, providing services as ordered by physicians within the plan of care.
  • Ensure that a Clinical Manager or Supervisor is available during all operating hours.
  • Coordinate communication with physicians, integrate orders (including medication orders), and update individualized plans of care.
  • Receive case referrals and accept those that the agency can serve.
  • Review patient information, including home visits, to determine home health care needs.
  • Assign appropriate home‑care personnel to each case as needed.
  • Attend conferences with physicians to discuss patient eligibility for services.
  • Evaluate each case by reviewing clinician services, case conferences, and records, guiding clinicians toward improved performance and quality service.
  • Review patient medical diagnoses, procedures, medications, and clinical course to inform planning.
  • Assist clinicians in establishing therapeutic goals, setting priorities, and developing plans of care.
  • Attend, lead, and document case conference meetings to facilitate coordination of care.
  • Participate in quarterly record reviews and communicate findings and recommendations to the Director of Nursing and agency personnel.
  • Assist in screening, interviewing, and recommending new agency personnel for employment.
  • Assist in orienting new agency personnel.
  • Support the Director of Nursing in planning, implementing, and evaluating continuing education programs.
  • Comply with professional standards and principles.
  • Participate in quality assessment and performance‑improvement programs.
  • Perform other duties delegated by the Director of Nursing or Branch Manager.
Qualifications & Requirements
  • Current licensure as a Registered Nurse, Physician, Physical Therapist, Occupational Therapist, Speech‑Language Pathologist, or Social Worker in the state of practice.
  • At least two years of supervisory or management experience in home health or a related healthcare setting (preferred).
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, and reliable vehicle or public transportation access.
Preferred Knowledge and Skills
  • Excellent time‑management, observational, and problem‑solving skills.
  • Outstanding verbal and written communication abilities.
  • Self‑motivated, able to work independently while collaborating as part of an interdisciplinary team.
Supervisory Responsibilities

May have supervisory responsibilities for employees performing similar or related work within the department.

Physical Demands and Work Environment

Must be able to stand, walk, sit, use hands, and drive, with the reasonable ability to lift over twenty (20) pounds. Significant portions of the role (over 50%) involve travel to client or patient locations via car or public transportation. Work may require evening, night, holiday, or weekend assignments.
