• Serve as Mental Wellness Coordinator for the youth; develop, train, execute, schedule, monitor and report with a trauma informed focus for services available and utilized for each youth and monitor progress. Identify, schedule, coordinate and supervise programs and services according to needs of the 5-Point Child Development Plan with holistic care in the areas of Emotional, Social, Spiritual, Physical and Educational.
  
  The services must support the client’s service plan and child’s plan of service requirements to include behavioral health, life skills programs, mental wellness, academic programs, and vocational and recreational activities, cultural competence, maintaining connections and contact with siblings and family visits.
• Read CPS Contract for clarity, comprehension and implementation for compliance.
• Exemplifies and promotes the mental wellness of the youth.
• Develop relationships with community partners to promote participation within our program.
• Demonstrates a willingness to set goals and assume ownership of responsibilities.
• Assists in the development, administration, coordination and production of programs that promote wellness and a healthy lifestyle for our youth.
• Coordinate, publish, and facilitate a monthly calendar for all wellness activities including youth therapeutic services, on-site and offsite wellness events, and youth recreation outings.
• Complete Continuous Quality Improvement Plan (CQIP) for each youth and data tracking to analyze the individual program success of the youth, make treatment plan suggestions, and track the performance of interventions used.
• Familiarize and communicate transitional support services available to the youth.
• Coordinate vocational classes for the youth in preparation for independent living.
• Conduct life skills group and nurture group with the youth.
• Work with Treatment Coordinators to schedule therapeutic services that help support child’s plan of service requirements.
• Ensures mental wellness equipment is sound and in working condition.
• Sends weekly reminders about upcoming events to staff.
• Meet with youth weekly to discuss any needs, issues, or concerns.
• Oversee implementation of activities, program schedules and functions to ensure they are met in a professional and timely manner.
• Responsible for managing and/or directing content for creating and implementing HWHF’s social media strategy to increase our online presence. This includes event photos, a monthly newsletter, engagement of our audience/community, and promoting our brand through social media platforms and HWHF website.
• Involved in event planning, marketing material organization, and supporting other marketing initiatives as needed.
• Assist youth in going to Texas Workforce Solutions office and Transition Centers and assist in enrolling in opportunities offered to the youth.
• Document all events and participation by youth for child’s plan of service.
• Teach therapeutic techniques to the youth that they can use for mental wellness.
• Ensure the privacy and confidentiality of all clients and their records.
• Complete duties in a professional and timely manner, meeting all deadlines.
• Assist in staff development and training programs to provide staff with information, tools and knowledge to help them effectively implement service plan requirements, i.e., behavioral tools, diagnosis, coping skills, and Contract requirements. Etc.
• Will provide transportation as needed.
• Attend monthly treatment team meetings and circle of support meetings.
• Other duties as assigned.
