What you’ll do- Responsible for the direct supervision of community-based non-certified clinical care staff.
- Provides training and education to care staff on an ongoing basis with classroom in-services and situation specific training.
- Supervises the maintenance of resident charts and reviews documentation performed by licensed and unlicensed care staff.
- As described and allowed in the Nurse Practice Act, assesses health, oversees medication administration, oversees functional and psycho-social status of residents, initiates individualized care plans, proactively manages care and services for each resident, evaluates effectiveness, and maximizes the resident’s opportunity to remain in their environment.
- Manages the healthcare of residents, including the delivery of pertinent care information to families and staff.
- Participates in the pre-admission process for new prospective residents. Ensures that all required documentation is completed prior to resident admission including nursing assessments, care plans and other associated forms.
- Documents and evaluates changes in condition and notifies Executive Director, resident physician and resident’s legally responsible party of resident’s condition.
- Provide oversight for community’s medication management program to ensure resident health and safety needs are met while maintaining compliance with state regulations.
- Coordinates with 3rd party providers to ensure continuity of care for residents receiving outside services such as hospice or home health.
- Shares on-call and manager on duty responsibilities/shifts as needed.
- Participates in department quality improvement activities.
- Participates in or leads meetings relevant to resident care issues including care plan meetings with appropriate parties as needed.
- Engages in departmental recruitment activities to maintain adequate staffing levels in the community.
- Other duties and projects as assigned.
- State Nursing License (LVN/LPN or RN)
- Board-approved degree or certificate in nursing, health sciences, or a related field
- Minimum of 3 years relevant experience, clinical leadership experience preferred.
- Experience working with Memory Care residents is preferred.
- Knowledge of RCFE Regulations
- Strong critical thinking and problem solving
- Technically savvy
- Patience and compassion when dealing with others, especially those who are sick, injured, elderly, or from different backgrounds.
