Halcyon Home seeks an experienced Home Health Physical Therapist to provide care to our geriatric clients in Tomball/Spring, TX and surrounding area. The role involves ongoing clinical assessment, developing and evaluating plans of care, documenting treatment, and providing patient and caregiver education.

• Perform ongoing clinical assessments, including OASIS assessments, to identify patient needs and determine level of functioning.
• Develop and evaluate plan of care in partnership with patient, representatives, and caregivers.
• Document clinical notes, phone orders, and other records on an ongoing basis and in a timely manner.
• Provide services ordered by the physician as indicated in the plan of care and oversee other team members when applicable.
• Apply infection control and standard precautions in patient care activities.
• Accept clinical assignments consistent with education and competence to care for patients.
• Provide supervision and guidance to physical therapy assistants and home health aides.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
• Communicate with all physicians and other health care practitioners involved in the plan of care.
• Meet mandatory continuing education requirements of the Agency and licensing board.
• Assist and instruct patients and families on transfer techniques and use of equipment such as walkers, wheelchairs, and canes.
• Provide patient, caregiver, and family counseling and education.

$75 - $150 hourly wage

• Current PT license
• Knowledge or experience utilizing Homecare Homebase EMR preferred
• Experience in home health or related healthcare setting
• Strong clinical and assessment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively.
