Pathways Hospice & Transition Home Healthcare is seeking a Social Worker (Home Health & Hospice) to support our mission and core values by addressing patients’ physical, mental and psychosocial needs. This PRN role involves admissions interviews, directive discussions with families, and collaboration with Nursing to ensure appropriate DNR orders.
Must hold a Bachelor’s or Master’s in Social Work and a Texas social work license, with at least two years in a supervisory hospice or related home
