Location: Spring Creek Village, an Independent and Assisted Living with Memory Care senior community
Pay Rate: $28.00/hr.
Full Time Shift: 6:00 a.m. – 2:00 p.m.
Must be available on weekends and holidays as needed.
Essential Duties and Responsibilities- Administer medications safely and accurately per physician orders and regulatory standards.
- Oversee medication aides and conduct monthly medication reconciliation and audits.
- Coordinate daily staff assignments to ensure consistent resident care.
- Communicate effectively with physicians, families, and care teams regarding resident needs.
- Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support.
- Respond to medical and behavioral emergencies with sound clinical judgment.
- Complete clinical documentation including assessments, evaluations, and service plans.
- Document incidents and communicate promptly with leadership.
- Monitor care team documentation for accuracy and completeness.
- Support orientation and ongoing training of care staff.
- Assist with scheduling in collaboration with leadership.
- Attend required in‑services and continuing education.
- Maintain professional appearance and hygiene.
- Promote respectful and effective communication across teams and families.
- Perform other duties assigned to support resident care and operational excellence.
- Current and valid LVN/LPN license in the state of employment.
- Basic Life Support (BLS) / CPR certification.
- Minimum 6 months to 1 year experience in Assisted Living or Long‑Term Care.
- Proficient in Electronic Health Records (EHR) and basic computer skills.
- Strong verbal and written communication skills.
- Demonstrated critical thinking and problem‑solving abilities.
- Knowledge of HIPAA regulations and compliance standards.
- Ability to respond with patience and understanding to dementia‑related behaviors.
- Physical ability to perform essential job functions with or without reasonable accommodation.
