Location: Spring Creek Village, an Independent and Assisted Living with Memory Care senior community

Pay Rate: $28.00/hr.

Full Time Shift: 6:00 a.m. – 2:00 p.m.

Must be available on weekends and holidays as needed.

Essential Duties and Responsibilities
  • Administer medications safely and accurately per physician orders and regulatory standards.
  • Oversee medication aides and conduct monthly medication reconciliation and audits.
  • Coordinate daily staff assignments to ensure consistent resident care.
  • Communicate effectively with physicians, families, and care teams regarding resident needs.
  • Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support.
  • Respond to medical and behavioral emergencies with sound clinical judgment.
  • Complete clinical documentation including assessments, evaluations, and service plans.
  • Document incidents and communicate promptly with leadership.
  • Monitor care team documentation for accuracy and completeness.
  • Support orientation and ongoing training of care staff.
  • Assist with scheduling in collaboration with leadership.
  • Attend required in‑services and continuing education.
  • Maintain professional appearance and hygiene.
  • Promote respectful and effective communication across teams and families.
  • Perform other duties assigned to support resident care and operational excellence.
Qualifications
  • Current and valid LVN/LPN license in the state of employment.
  • Basic Life Support (BLS) / CPR certification.
  • Minimum 6 months to 1 year experience in Assisted Living or Long‑Term Care.
  • Proficient in Electronic Health Records (EHR) and basic computer skills.
  • Strong verbal and written communication skills.
  • Demonstrated critical thinking and problem‑solving abilities.
  • Knowledge of HIPAA regulations and compliance standards.
  • Ability to respond with patience and understanding to dementia‑related behaviors.
  • Physical ability to perform essential job functions with or without reasonable accommodation.
