As an LVN Home Care Nurse, you'll work closely with physicians, caregivers, and healthcare professionals to deliver individualized care plans that meet each client's unique needs. You'll assess patient conditions, administer medications and treatments, monitor health status, educate patients and families, and communicate important clinical updates to the care team. Your clinical judgment and compassionate approach will play a vital role in helping patients remain safe and comfortable at home.

We're seeking a dedicated LVN with an active Texas nursing license, current in-person CPR/BLS certification, valid Texas driver's license, current automobile insurance, and a recent TB test or chest X-ray. Candidates should have at least one year of nursing experience in a home health agency, healthcare facility, primary care clinic, or similar setting, along with a strong commitment to providing high-quality, patient-centered care.
